29 CFR 1926.20(b)(2): Frequent and regular inspections of the job site, materials, and equipment to be conducted by a competent person designated by the employer was not provided by the employer who was responsible to initiate and maintain an accident prevention program:    a- 626 Gravelly Hollow Road, Medford NJ: A competent person did not conduct frequent and regular inspections of the jobsite.  Employees on this site were exposed to hazards including, but not limited to fall hazards.  Condition noted on or about 07/14/16.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:    a- 626 Gravelly-Hollow Road, Medford NJ: Employees, not utilizing fall protection, were exposed to fall hazards in excess of 13 feet to the ground surface below when performing roof shingle rip, tear, replace, and other related activities from a commercial building's roof with a pitch of approximately 4 on 12.  Condition observed on or about 07/14/16.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a- 626 Gravelly-Hollow Road, Medford NJ: Employees, not utilizing fall protection, were exposed to fall hazards in excess of 13 feet to the ground surface below when performing roof shingle rip, tear, replace, and other related activities from a commercial building's roof with pitches ranging from approximately 6 on 12 to 8 on 12.  Condition observed on or about 07/14/16.
