29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures was not permanent and continuous:    (a) Building 704, Mechanical Room:  A GFCI Unit Catalog No. 30338 was missing a grounding prong and was used to energize a 110 VAC Pullman-Holt vacuum cleaner. The vacuum cleaner was not designed with double insulated wiring, exposing workers to electrical shocks.
