29 CFR 1910.30(a)(2): Portable dockboards were not secured in position, either by being anchored or equipped with devices which would prevent their slipping:   A.  On or about July 17, 2016, Bay #2:  Toyota Electric Forklift Truck, Model 7FBEU15, Serial Number 20126 was driven onto the back of a straight delivery truck with a Gross Vehicle Weight of 16,000 pounds.  The portable dock plate used at the bay was not secured.  The forward motion of the powered industrial vehicle caused the delivery truck to roll away from the dock, causing the dock plate to slip and the forklift and pallet of airplane tires to fall three feet eleven inches to the ground below.
