29 CFR 1926.102(a)(1):   Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On 7/18/2016, employees from JCR Contracting LLC were installing shingles on a roof of a residence located at 1011 Glengarry Drive, Wentzville MO.  The crew leader and two employees were not wearing eye protection while using pneumatic nail guns.

29 CFR 1926.501(b)(13):   "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 7/18/2016, employees from JCR Contracting LLC were installing shingles on a roof of a residence located at 1011 Glengarry Drive, Wentzville MO.  They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   The crew leader and four employees were observed on the roof of the building without fall protection, exposing them to a fall to the ground below.

29 CFR 1926.1053(b)(1):   When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  On 7/18/2016, employees from JCR Contracting LLC were installing shingles on a roof of a residence located at 1011 Glengarry Drive, Wentzville MO.  The employees were using three ladders to access the roof.  Two of the three ladders were not extended three feet beyond the landing or edge of the roof.
