(a) Truck #74, one (1) employee working from a truck mounted aerial lift was not wearing a fall restraint system or personal fall arrest system. Employee was exposed to a fall hazard of 38-feet.     (b) Truck # 95, one (1) employee working from a truck mounted aerial lift was not wearing a fall restraint system or personal fall arrest system. Employee was exposed to a fall hazard of 38-feet.     (c) Truck # 142, one (1) employee working from a truck mounted aerial lift was not wearing a fall restraint system or personal fall arrest system. Employee was exposed to a fall hazard of 38-feet.

29 CFR 1926.957(b)(3): Live-line tools used for primary employee protection were not removed from service every (2) years, or whenever required under paragraph (b)(2) of this section:     (a) Truck #95, one (1) employee was performing work on an energized overhead power line from a truck mounted aerial lift was working with a hot stick, and the employer did not ensure the live-line tools were removed from service when the insulating tool showed visual evidence of being overstressed or damaged.
