29 CFR 1926.503(b)(1):     The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.    a)  Springfield Church of the Brethren - On or about July 20, 2016, employees are working on low pitch roof, up to 14 feet above grade, without utilizing fall protection system.

29 CFR 1926.1060(a):     The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:    a)  Springfield Church of the Brethren - On or about July 20, 2016, the ladder used by employees to access the roof, up to 14 feet above grade, does not extend at least three feet above the surface.
