29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a. Cambridge Lofts, 1133 North 4th Street, Philadelphia, PA 19123 - The employer did not ensure employees, who were working near unguarded roof openings were protected from falling. An employee fell through a hole and sustained a broken left hip. Employees were exposed to a fall hazard of approximately thirteen (13) feet, three (3) inches. Condition observed On or About 07/19/16.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a. Cambridge Lofts, 1133 North 4th Street, Philadelphia, PA 19123 - The employer did not provide training to employees who were exposed to fall hazards. Employees were installing skylights and were exposed to fall hazards of approximately thirteen (13) feet, three (3). An employee fell through a roof hole and sustained a broken left hip. Condition observed On or About 07/19/16.
