29 CFR 1926.352(a): Movable fire hazards in the vicinity of unmovable objects that were to be welded on, cut or heated were not taken to a safe place, or otherwise protected:  a) On or about July 18, 2016 and at times prior thereto employees were welding in an aerial lift, where a 100 percent cotton tarp, lumber, and other combustibles were contained within the aerial lift. 29 CFR 1926.352(d): Suitable fire extinguishing equipment was not immediately available in the work area(s) where welding, cutting, or heating was being performed:  a) On or about July 18, 2016 and at times prior thereto employees were welding in an aerial lift, where an inadequate Class 10-B/C fire extinguisher was used to attempt to extinguish combustible galvanized steel, a cotton tarp, welding lead coating, and various pieces of wood.  NOTE-Abatement certification and documentation are required for this item.

29 CFR 1926.354(c)(2): Employee(s) were not protected by a respirator, in accordance with the requirements of 29 CFR 1926 Subpart E, when welding, cutting or heating toxic preservative coatings in open air.    a) On or about July 18, 2016 and at times prior thereto employees were welding galvanized coated steel, from an aerial lift outside under an overhang, where no respirators were available or used. No assessment of worker exposure to hazardous chemicals, such as zinc fume, had been conducted.    NOTE-Abatement certification is required for this item.
