29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:  North side; JLG Aerial Lift (Model: 400S, Serial No:0300116984), On or about July 20, 2016, an employee without fall protection was exposed to fall hazards, while installing metal frame supports.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  On the roof of a commercial building, North side, on or about July 20, 2016, an employee without fall protection was exposed to fall hazards greater than 6 feet to the paved ground below, while cutting framing studs.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about July 20, 2016, employees without fall protection were exposed to fall hazards, while installing framing studs on a commercial building under construction without being trained to recognize fall hazards.
