29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a) Whitemarsh Shopping Center, Butler and Ridge Pikes, Conshohocken PA - On or about July 19, 2016, employees engaged in roofing activities on a flat roof were not protected from falls greater than 20 feet to ground in that the employees were working outside of the warning line system.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder  (the distance along the ladder between the foot and the top support).  (a) Whitemarsh Shopping Center, Butler and Ridge Pikes, Conshohocken PA - On or about July 19, 2016, employees accessed a roof from a ladder which was more than one-quarter of the working length of the ladder, in that the foot of the 18 foot working section of an aluminum extension ladder was measured at approximately 6 feet from the building.
