29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled:     a)  On or about July 20, 2016, an Expedited Freight Systems, LLC driver assisted host company forklift operator to unload a "Dyne" unit from a semi-truck trailer.  The driver was in direct contact with the "Dyne" unit, exposing the employee to struck-by and crushing hazards while being moved by the forklift operator.
