29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever necessary by reason of hazards of processes or environment:        a)  Employees did not have hearing protection when operating compressed air jackhammers and a larger machine mounted jackhammer; employer was demolishing a 2 story parking garage, jackhammering concrete floors and columns. 29 CFR 1926.350(a)(9): Compressed gas cylinders were not secured in an upright position at all times, accept, if necessary, for short periods of time while cylinders are actually being hoisted or carried:      a)   Two compressed gas cylinders (oxygen and acetylene) were standing unsecured on the 2nd floor,  while employees were working on the demolition of a building. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which is 6 feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      a)  Employees were exposed to falling 15 ft while operating jackhammers at floor edges without utilizing any form of fall protection system.      b)  Employee was exposed to falling 15 ft standing on the building structure edge doing cement coating without utilizing any form of fall protection system.
