29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):      a)    At the 2 Roseberry Circle, Pt. Wentworth jobsite: On July 21, 2016, and at times prior thereto, employees were exposed to fall hazards of up to 28 feet,6 inches, in that roofers without fall protection were engaged in shingle-application activities on a 7:12-slope roof.       b)    At the 2 Roseberry Circle, Pt. Wentworth jobsite: On July 21, 2016, and at times prior thereto, an employee was exposed to fall hazards of about 8 feet, in that a roofer without fall protection was engaged in connecting an air compressor on a 4:12-slope porch roof.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:      a)    At the 2 Roseberry Circle, Pt. Wentworth jobsite: On July 21, 2016, and at times prior thereto, employees were exposed to fall hazards of up to 17 ft. 4 in, in that a 32-ft. portable ladder was used with side rails not extend at least three (3) feet above the upper landing, was not secured at the top, and a grasping device was not provided.

29 CFR 1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards:  a)   At the 2 Roseberry Circle, Pt. Wentworth jobsite: On July 21, 2016, and at times prior thereto, employees were exposed to fall hazards of up to 17 ft. 4 in, in that roofers were not trained to recognize ladder hazards and procedures to minimize these hazards as they used a 32-ft. portable ladder to access and egress a steep-slope roof.

Wentworth jobsite: On July 21, 2016, and at times prior thereto, employees were exposed to eye hazards, in that roofers did not wear eye or face protection while engaged in securing shingles using a pneumatic nail gun.
