29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures was not permanent and continuous: a) 2 Grafton Commons., Grafton, MA: On or about July 19, 2016, employees were using power tools that were powered through an extension cord; which was plugged into an adapter made to circumvent the grounding pin on the extension cord. That made the path to ground not continuous from the circuit to the tool they were using to perform their work.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) 2 Grafton Commons., Grafton, MA: On or about July19, 2016, an employee was painting trim on the mansard roof of the building at this address without any fall protection, more than 20 feet above the ground below. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
