Dupont Highway, old K-Mart store, Dover, DE - On or about July 22, 2016, the employer failed to ensure employees used a safety harness with lanyard attached to the basket to conduct EFIS/stucco work approximately 15 feet above the grade, exposing the employees to an ejection hazard.

29 CFR 1926.453(b)(2)(iv): Employees did not always stand firmly on the floor of the basket, and climbed on the edge of and out of the basket for a work position:    a) 515 N. Dupont Highway, old K-Mart Store, Dover, DE: On or about July 22, 2016, employees conducting EFIS/Stucco activities from an aerial lift were standing on the guardrail system and climbed out of the basket.
