29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:   a) Located at The Manor, 901 West Main Street, Freehold, NJ: On or about 7/5/16, an employee passed out while working in the kitchen and was hospitalized. The employer reported the in-patient hospitalization to OSHA on 7/13/2016, not within the required 24 hours.
