Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    a. On or about July 18, 2016, at the JC Penney store located at 288 Orland Square Drive, Orland Park, Illinois an employee required to access and work on top of a semi trailer was exposed to falls of approximately ten (10) above the ground level.  The employer did not ensure that employees used protective equipment, such as a body harness and lanyard, to protect them against injury in the event of a fall.

The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    a. On or about July 18, 2016, in the JC Penney Store at 288 Orland Drive, Orland Park, IL , an employee was required to perform maintenance activities including but not limited to changing fuses in electrical fixtures. The employer did not have a written lockout program which included energy control procedures, periodic inspections and employee training.  The employees were thereby exposed to the hazards associated with unexpected energizing of electrical equipment.
