On 7/26/16, at the Peachtree Woodworking Supply, Inc. facility, the employer used a job made man lift platform on a Clarke sit down propane powered forklift for gaining access to warehouse racking shelves. The platform is a pallet with a flat table secured to the pallet. The platform has not been approved by the manufacturer and was not secured to the forklift. The platform was not equipped with guard rails and no other fall protection is used to prevent employees falling as they access shelving and are exposed to >10 fall hazards.
