29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   (a)   On or about 05/25/16, at the Market Commons, located at 100 Market Place Drive, in Rochester, New York; an employee was working from an aerial lift (Genie S-40), without "tying off" or anchoring the harness lanyard to the boom basket.  Employee was 10 feet above ground level.  NO ABATEMENT CERTIFICATION REQUIRED
