29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person:  35 Meadow Road, Chicopee, MA: On or about July 27, 2016 an employee was observed working from a carpenter bracket scaffold which was not constructed in accordance with the design by a qualified person (manufacturer) exposing employees to falls over 11 feet.  (a) Rear of building, corner.  Support bracket did not have a fender washer installed with the nut to prevent pull out from bracing. (b) Brackets were spaced more than 20 feet apart which is more than the required 8 feet between brackets posing an overload hazard.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
