29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) Wilkeswood Apartments, Wilkes Barre, PA: An extension ladder that was used by 8 employees to access the roof of an apartment building was not properly installed in that the side rails did not extend at least three feet above the eave. This could have resulted in the ladder being dislodged and exposure to a fall, observed on or about July 28, 2016.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Wilkeswood Apartments: Employees performing roofing work on an apartment building with an eave height of approximately 17 feet were not protected from falling, on or about July 28, 2016.   ABATEMENT CERTIFICATION DOCUMENTATION REQUIRED  Josh Thomas was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 982743, citation number 1, item number 1, and was affirmed as a final order on August 29, 2014, with respect to a workplace located at 12 Erin Drive, Danville, PA.
