Employees were not provided with personal protective equipment including, but not limited to personal fall arrest system(s) when exposed to fall hazards.

On or about July 27, 2016, at the above addressed workplace, employees were exposed to fall hazards when required to access and perform work on customer roof(s) to service equipment.  The employer failed to adequately assess hazards which were present and/or likely to be present which necessitated the use of personal protective equipment including but not limited to personal fall arrest systems.
