29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:    a.)    Alpha Sintered Metals, LLC., 95 Mason Run Rd., Ridgway, PA 15853, front of S-19 sintering oven, on or about July 28th, 2016, and times prior thereto:  The flexible electrical cord used to provide electrical power to the Global industrial 30" pedestal fan (FS-75x Model: 585280) was frayed on the outer jacket and contained a damaged strain cap.

Alpha Sintered Metals, LLC., 95 Mason Run Rd., Ridgway, PA 15853, on or about July 28th, 2016, and times prior thereto:  The OSHA Form 300 for calendar year 2016 did not provide specific detail, about the parts of the body that were affected, in Column F.  Case numbers 2 and 3 did not describe the object/substance that directly injured or made the person ill.
