29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a)  ADA Dash Subline - A06-1/A06-2;  On or about June 18, 2016 and at times prior thereto, where procedures had not been developed, documented, and or utilized for the control of potential hazardous energy, Employee #2 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter a robotic cell where there were energized robots and other jigs and fixtures to replace a welding tip.  b)  ADA Dash Main Line - A900;  On or about June 18, 2016 and at times prior thereto, where procedures had not been developed, documented, and or utilized for the control of potential hazardous energy, Employee #9 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter a robotic cell where there were energized robots and other jigs and fixtures to replace a welding tip.  c)  JFA Quarter Line L/H - A06/A101/A05;  On or about June 17, 2016 and at times prior thereto, where procedures had not been developed, documented, and or utilized for the control of potential hazardous energy, Employee #28 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter a robotic cell where there were energized robots and other jigs and fixtures to replace a welding tip.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:   a)  ADA Dash Subline - A06-1/A06-2;  On or about June 18, 2016 and at times prior thereto, Employee #2 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter the robotic cell with energized robots and other jig and fixtures to replace a welding tip on a robot. The employee was unable to lock out the robotic cell because the employer had not provided or ensured the employee had a lock.  b)  ADA Dash Main Line - A900;  On or about June 18, 2016 and at times prior thereto, Employee #9 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter the robotic cell with energized robots and other jig and fixtures to replace a welding tip on a robot.
