29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:  a)  At Front of House:  On or about July 21, 2016 and at times prior; the employer exposed his employees to a fall hazard of 14 feet 6 inches to the ground below in that he allowed his employees to work from a scaffold walk board which overhung by three feet creating a cantilever.  b)  At Side of House:  On or about July 21, 2016 and at times prior; the employer exposed his employees to a fall hazard of 14 feet 1 inches to the ground below in that he allowed his employees to work from a scaffold walk board which overhung by three feet creating a cantilever.

29 CFR 1926.452(j)(1): Pump jack brackets, braces, and accessories shall be fabricated from metal plates and angles. Each pump jack bracket shall have two positive gripping mechanisms to prevent any failure or slippage:  a)  At Side of House:  On or about July 21, 2016 and at times prior; the employer allowed his employees to work from a pump jack scaffold which had only been braced at the top portion  of the scaffold exposing employees to a scaffold collapse and a fall hazard of 14 feet 1 inch to the ground below.
