29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:   On or about August 1, 2016, an employee was exposed to a fall hazard by standing on a temporary elevated work platform partially attached to a forklift that was being driven through the parking lot of the commercial construction site.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 1, 2016, employees were exposed to a fall hazard by using a ladder that did not extend 3 feet above the upper landing surface to access the roof of the commercial construction site.
