Building or structure exits were not so arranged and maintained as to provide free and unobstructed egress from all parts of the building or structure at all times when it was occupied.    a) Jobsite entrance, 1st floor:    A lock was installed on the entrance door which prevented free escape from the inside of the building.  Employees were locked inside the site.   Demolition debris was stored in the building such that it obstructed egress from the 1st floor when employees worked on site. On or about 07/29/16.
