29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    Employees working in the food distribution warehouse and engaged in fork truck battery maintenance activities were exposed to health hazards associated with contact with corrosive chemicals.  An eyewash station was not provided within the work area for immediate emergency use.

The employee operating the Crown, stand-up style fork truck, had not been provided training or an evaluation regarding its safe operation.

29 CFR 1904.39(a)(2):  The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four hours:  The employer did not report the accident which occurred on July 21, 2016 until July 26, 2016.  The fork truck accident resulted in the in-patient hospitalization of an employee.
