29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:    Employees were exposed to falls to the concrete floor surface at the following:    a) Varnish Production Area at the West end beneath the mixers (1-4) and the floor was coated with wet varnish spilled from the mixers.    b) Storage Building at the East pile of 55-gallon drums containing recycled oil and ethyl acetate and one drum had two pinhole leaks with the spill absorbent left on the ground.

29 CFR 1910.305(j)(2)(iv): A receptacle installed in a wet or damp location was not suitable for the location:    The employer provided electrical receptacles beneath the mixers along the West wall of the plant and the receptacles were not intended to protect against contact with varnish.
