29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.    Cedar Valley Steel Contracting LLC project  site located at 1117 Washington Ave, Appleton, WI: On August 2, 2016, employees were exposed to falls while using a Genie Z-34/22 articulating boom lift to access the roof of the building without using harnesses with lanyards to prevent them from being ejected.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  Cedar Valley Steel Contracting LLC project site located at 1117 Washington Ave. Appleton, WI: On August 2, 2016, employees reroofing an engineered steel building were exposed to falls when no perimeter warning line was used to mark the edges of the roof.
