29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  (a) Rear of the Building - The employees accessed the 15 foot high roof by way of an Louisville, fiberglass, extension ladder that passed 54 inches under the energized service drop lines going into the building, exposing the employees to a hazard, observed on or about 8-03-2016.  b)  Rear of the Building - The employer supported a metal pipe to convey the hot asphalt to the roof with a Louisville, fiberglass, extension ladder that passed about 1 to 1 1/2 feet under the electrical service line for the building, exposing employee to a hazard, observed on or about 8-03-2016.   Abatement Certification Required.
