29 CFR 1910.23(c)(1): 29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3) of this section) on all open sides:    Location: Loading dock, Bay #4    On or about 7/21/16, an employee standing on the dock plate with a loaded bossy cart, fell approximately 4-feet, 1-inch to the ground below when the truck driver moved the trailer being loaded without verifying that trailer was finished being loaded.

29 CFR 1910.178(n)(11): Dockboard(s) or bridgeplate(s) were not properly secured before they were driven over:    Location: Bays #5 and #6      On or about 7/28/16, the employer retrofitted an enclosed trailer to be a loading dock and did not level it causing the dock boards to not be secured in place when powered pallet jack operators went over the dock boards exposing the employees to crush-by hazards.
