(b) Pro Tank Products Shop: On August 3, 2016 and at times prior employees were exposed to skin abrasions and lacerations while using an unguarded Bosch model portable grinder.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use: (a) Pro Tank Products shop: On August 3, 2016 and at times prior employees were exposed to struck by hazards in the event of compressed gas bottles being tipped over. Compressed gas bottles such as, but not limited to Argon, Nitrogen, Oxygen, and Carbon Dioxide were stored without safety caps in the shop.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose; and each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: (a) Northern bay Pro Tank Products shop: On August 3, 2016, and at times prior, employees were exposed to electrical shock hazards due to not having a faceplate covering a wall outlet on the north side of the shop. (b) Northern bay Pro Tank Products shop: On August 3, 2016 and at times prior, employees were exposed to electrical shock hazards due to an energized electrical panel junction box with the covers removed.
