29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Second Floor Balcony, Rexburg, Idaho: On or about August 3, 2016, the employer did not ensure employees were protected from fall hazards while working on unguarded balconies and exposed to fall hazards ranging from heights of approximately 13-18 feet from the ground surface below.    Note) Abatement certification IS required for this item.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee had been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:  a) Jobsite: On or about August 3, 2016 and times prior thereto the responsible person on site and employees performing work on unguarded balconies did recognize the hazards associated without any means of fall protection and take actions to minimize or eliminate these hazards.    Note) Abatement certification IS required for this item.
