On or about August 4, 2016, employees were operating a JET horizontal/vertical bandsaw, model HVBS-56M, serial 150640855. The unused portion of the cutting blade was unguarded, exposing employees to an amputation hazard.

The saws yoke assembly did not return once released by the operator, exposing employees to an amputation hazard.
