29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On or about August 2, 2016, at the worksite located at 1315 East Cadron Gap Road in Conway, Arkansas, employees using pneumatic nail guns to fasten shingles to the roof did not wear eye protection.  This condition exposed employees to the hazard of eye injuries.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about August 2, 2016, at the worksite located at 1315 East Cadron Gap Road in Conway, Arkansas, east side of residential home being constructed, the portable extension ladder used to access and descend from the roof did not extend 3 feet above the landing surface.  This condition exposed employees to falls from elevations.

This condition exposed employees to the hazard of falls from elevations.
