29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about August 5, 2016 at the jobsite located at or near 3103 NE 79th Terrace, employees were exposed to a struck by hazard in that eye protection was not provided while using a pneumatic nailer.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On August 5, 2016 at the jobsite located at or near 3103 NE 79th Terrace in Kansas City, MO employees were exposed to a fall hazard in that they were working on a steep roof approximately 18 feet above the ground level without an approved form of fall protection.
