29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition       (a) On or about August 5, 2016, at the Best Service Stores facility #156-4 in Kansas City, MO., employees were exposed to serious injuries, in that the Toyota Powered Industrial Truck , Serial Number #13881 was in operation with an inoperable/defective emergency brake. When tested, the emergency brake would not operate and hold as required in neither forward or reverse gear.      Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
