29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety.    (a) On or about August 8, 2016 for employees exposed to cave-in hazards while installing a sewer line in a trench with a depth of up to 10 feet where adequate protective systems were not in place at the project site located near 1440 Action Drive in Mandan, North Dakota.    Note:  This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 1150883, Citation  1, Item  1a, and was affirmed as a final order on August 15, 2016, with respect to a workplace located at 2500 Overlook Lane, in Mandan, North Dakota.    Note:  This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number  1135841, Citation  1, Item  1b, and was affirmed as a final order on June 21, 2016, with respect to a workplace located at 3526 Clairmont Road in Bismarck, North Dakota.
