29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees:  The employer is failing to protect employees of the company from fall hazards.  The cover on the floor hole on the second floor of the facility being renovated was not secured to ensure that it could not be moved when not in use.  This allows employees to inadvertently move/remove the cover creating a fall hazard.  One instance, which occurred on or about August 3, 2016, involved a contractor worker falling through the floor hole in the facility located at 2100 E. Overland Dr., Scottsbluff, Nebraska.
