29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures:  JOB SITE: 5407 Guinevere Drive, Weldon Spring, MO 63304, on or about 8/9/2016, workers were observed on top of a 10:12 pitch roof, approximately 21 feet 5 inches eave to ground level, actively installing new shingles, and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  JOB SITE: 5407 Guinevere Drive, Weldon Spring, MO 63304, on or about 8/9/2016, a worker was observed using a blue fiberglass extension ladder, where the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  The worker was exposed to a fall hazard.
