29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration: Such activities were not performed only by experienced and trained employees selected for such work by the competent person.    Jobsite:  Employees were injured while working from ladder-jack scaffolding that had not been erected by or under the supervision of a competent person when the ladder-jack scaffolding shifted causing the employees to fall approximately twelve (12) feet to the ground below.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    Jobsite:  Employees were exposed to a fall of approximately twelve (12) feet while working on ladder-jack scaffolding without the use of fall protection.

29 CFR 1926.452(k)(4): Ladders used to support ladder-jacks were not placed, fastened, or equipped with devices to prevent slipping:    Jobsite:  Employees were working approximately twelve (12) feet above the ground on a ladder-jack system where the ladders were not placed or fastened to prevent slipping.  Two employees were seriously injured when a ladder slipped causing the employees to lose balance and fall to the ground.

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another.    Jobsite:  Employees were working from a ladder-jack scaffold system where three ladders were used to support two scaffold planks.  There was a wooden plank bridged on top of an aluminum plank.  The ladder in the middle supporting the section that was bridged shifted and two employees were seriously injured when they fell approximately twelve (12) feet to the ground below.
