Jobsite: Employees were injured while working from ladder-jack scaffolding that had not been erected by or under the supervision of a competent person when the ladder-jack scaffolding shifted causing the employees to fall approximately twelve (12) feet to the ground below.

Two employees were seriously injured when a ladder slipped causing the employees to lose balance and fall to the ground.

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another. Jobsite: Employees were working from a ladder-jack scaffold system where three ladders were used to support two scaffold planks. There was a wooden plank bridged on top of an aluminum plank. The ladder in the middle supporting the section that was bridged shifted and two employees were seriously injured when they fell approximately twelve (12) feet to the ground below.
