29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On or about August 9, 2016 at worksite 9426 Smalley Ave, Kansas City, Missouri.- An employee was exposed to a fall hazard when performing roofing activities at an elevation of approximately 20 feet above the surface below without an approved form of fall protection to include, guardrail systems, safety net systems, or personal fall arrest systems. Abatement Note: Abatement certification for this violation is not required.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) ) On or about August 9, 2016 at worksite 9624 N Smalley, Ave, Kansas City, Missouri.- An employee was exposed to a fall hazard by using a ladder which did not extend 3 feet above the landing surface of the roof to which he gained access.
