On or about 08/08/16, at the site of 41-23 Benham Street. Elmhurst, NY     Employees working below a pump jack scaffold where other employees were removing siding and gutters 16-ft. overhead were exposed to struck-by hazards. The employees were not provided protective helmets.      ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

Elmhurst, NY     Employees removing sidings and gutters, 16-ft. above a lower level, on a pump jack scaffold that did not have a midrail installed.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
