29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   On or about August 8, 2016, the employer did not ensure hazardous energy control procedures were utilized for the safe application, usage, and removal of energy controls when employees changed the slitter knives (set-up) on the Paxson Machine Co. 60 Slitter Line.  Energy Sources include, but are not limited to, electrical, pneumatic, and hydraulic power.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   On or about August 8, 2016, machine operator(s) and machine helper(s) were exposed to machine hazards associated with rotating parts during the changing of the slitter knives (set-up) of a Paxson Machine Co. 60 Slitter Line.  The employer did not ensure machine operator(s) and machine helper(s) implemented energy control procedures by isolating the machine from the energy source(s) [(d)(3)], affixing lockout device(s) [(d)(4)(i)], and verifying de-energization and isolation had been accomplished prior to setting up the slitter knives [(d)(6)].  Energy Sources include, but are not limited to, electrical, pneumatic, and hydraulic power.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about August 8, 2016, the employer did not ensure one or more methods of machine guarding were provided to protect machine operator and helpers from the hazards created by the ingoing nip points, rotating parts and moving steel coil material of the Paxson Machine Co. 60 Slitter Line.  The unguarded rotating parts, ingoing nip points, and point of operation include; un-coiler/pay-off reel, guide rolls, slitter knives, material moving through the machine, scrap winders, re-coiler reel, and over-arm.
