29 CFR 1926.1203(a): Before it begins work at a worksite, each employer must ensure that a competent person identifies all confined spaces in which one or more of the employees it directs may work, and identifies each space that is a permit space, through consideration and evaluation of the elements of that space, including testing as necessary.  a.  On the site where employees were removing equipment from a confined space electrical vault, the competent person did not identify the working space as a confined space and evaluate potential hazards relating to working in the confined space thereby exposing employees to a electrical hazard.

29 CFR 1926.1203(e)(2)(iii): (iii) Before an employee enters the space, the internal atmosphere must be tested, with a calibrated direct-reading instrument, for oxygen content, for flammable gases and vapors, and for potential toxic air contaminants, in that order. Any employee who enters the space, or that employee's authorized representative, must be provided an opportunity to observe the pre-entry testing required by this paragraph.  a.  On the site where employees entered a Confined Space to remove equipment from the electrical vault, the employer did not test the atmosphere for oxygen content, flammable gases and vapors and potential toxic air contaminants before allowing employees to enter the confined space thereby exposing employees to unknown hazards.

29 CFR 1926.1203(e)(2)(ii): (ii) When entrance covers are removed, the opening must be immediately guarded by a railing, temporary cover, or other temporary barrier that will prevent an accidental fall through the opening and that will protect each employee working in the space from foreign objects entering the space.  a.  On the site where employees were working in a confined space to remove equipment, the attendant was exposed to a 14 foot fall hazard while standing at the opening where employees entered the electrical vault.
