On or before Thursday, August 11, 2016 at 115-40 Dunkirk Street, St. Albans, NY 11412 (Warehouse)    Warehouse employees are required to stand on elevated wooden platforms attached to the forks of powered industrial trucks in order to retrieve boxes and containers containing ceramic floor/wall tiles at a height of approximately 16 feet above the lower level without a means of fall protection provided exposing employees to fall hazards.    ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
