29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l).  a) The employer, Gross Printing Associates, did not certify that each operator had been trained and evaluated as required by paragraph (l). An employee operating a powered pallet jack adjacent to the loading dock of the facility operated the pallet jack in such a manner that allowed the pallet jack to ride over the employee's leg, on or about August 12, 2016.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
