29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  The employer is failing to ensure that employees are protected from eye injuries.  The employees are exposed to flying debris hazards during the course of reroofing and applying shingles which can cause permanent and irreversible illnesses.  This most recently occurred on August 16, 2016 on the jobsite located at 8859 Venice Lane, Lincoln, NE 68526.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)    The employer is failing to ensure that employees exposed to fall hazards while working on roofs at heights greater than 6 feet at the location are utilizing fall protection.  The residential home has a roof pitch of 8:12 and the ground to eave height is approximately 10 feet.  This most recently occurred on August 16, 2016 on the jobsite located at 8859 Venice Lane, Lincoln, NE 68526.
