Emmaus Ave. Allentown, PA 18103, Roof: On or about August 8, 2016, employees were performing residential roofing work, approximately 13 feet above grade, without utilizing any means of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 911 W. Emmaus Ave. Allentown, PA 18103, Roof: On or about August 8, 2016, employees were using an aluminum extension ladder to access the roof of a residential home project, but the rungs did not extend at least three feet above the upper landing surface, exposing employees to a fall hazard of approximately eight feet.
